Adapting a template to your team
Start by cutting, not adding. Copy the full question set into your survey tool, then remove any question that doesn't map to a decision you're prepared to act on this cycle. A ten-question survey people finish beats a twenty-question survey half of them abandon.
Keep the confidentiality language in the opening instructions close to the original wording. Small changes to how you describe anonymity change how honestly people answer the rest of the survey.
Order matters. Put the questions you most need answered near the top. If your platform reports partial completions, someone who quits at question twelve still gave you usable data on questions one through eleven.
When to shorten it
Run the full template for an annual review, when you have time to read open-text answers and act on what you find. Cut it down to the core question block for a quarterly pulse, when speed matters more than depth.
Drop demographic fields like department and tenure on any team small enough that an answer could be traced back to one person. If you keep them, use broad bands such as under 1 year, 1 to 3 years, and 3-plus years instead of exact numbers.
